the Colonel's Bequest
This is a tough one. I've always loved the Phantas design, but to be honest it doesn't have much to do with the game. Colonel, on the other hand, instantly sets up the atmosphere and story. You can infer what the game is about with one quick glance, whereas Phantas just says "this is a horror game."
I love Phantas, but I gotta go with Colonel.

Colonel's Bequest.
Phantasmagoria is more iconic (at least to me), but it's an awful game. Colonel's Bequest might be less iconic, but the art is *better* and the game is *much better*.

Space Quest IV, easy. The plunger on that plump cheek is adorable. The sound effect seals the deal.
I might have mentioned this previously, but I have very distinct memories of buying this game at KB Toys with the $20 my grandmother gave me for my birthday. I was in third grade. My dad had given me the day off of school to go to the Liberty Science Center in NJ. On the way back, we stopped at the toy store because I wanted to buy this San Diego Zoo virtual tour cd-rom which had been advertised as being 'just like being at the zoo'. They didn't have that anymore, thank goodness, so I ended up with KQ IV!
Then again, my sister named her son Gabriel after Gabriel Knight. Before he was born, she told me she had chosen a name based on an adventure game character she used to have a crush on. I guessed George and Ben. I was wrong.
